Our Homecare program lets you recover in the place you feel best. If you're recuperating from an illness or healing after a surgery, our nurses and therapists can work with your physicians to design the recovery program that will help you achieve the best level of wellness possible.
We offer services that tend to the physical, behavioral and emotional health of our patients. Here are some of the ways in which the Farmington Valley Visiting Nurse Association (FV-VNA) can help you.
Registered nurses provide skilled nursing care based upon the plan of care established by your physician. Services are available seven days per week, twenty-four hours per day, 365 days per year.
Registered physical therapists and licensed occupational therapists provide therapy and exercises as ordered by your physician to help regain mobility, strength and ability to manage activities of normal living.
Certified and licensed speech and language therapists help retrain impaired speech, and provide assistance with swallowing disorders and other specialized therapy pertaining to speech and language deficits.
Licensed medical social workers provide supportive counseling to patients and families who are coping with illness, assistance to access community resources, and assistance with obtaining financial resources as needed.
Registered dieticians provide teaching and guidance regarding special diets, nutrition, meal planning/preparation and other diet-related issues.
Homemakers do light housekeeping, laundry, and aid with grocery shopping and meal preparation. Certified home health aides provide assistance with bathing, dressing, prescribed exercises, feeding and other personal care as instructed by the registered nurse or therapist.
Home Health Aides provide private duty care for two-hour shifts or more attending personal care, bathing, meal preparation, light housekeeping and companionship needs.
Successful discharge from the hospital is only the first step along the path to recovery. For patients recovering at home from an illness or a surgery, the FV-VNA's health supervision services offer minimum assistance to help foster continued good health.
